Primary Sclerosing Cholangitis (PSC)

General features
• It is a pathology with progressive inflammation, fibrosis and obstruction of the intra and extrahepatic bile ducts.
• The most common pathology is ulcerative colitis.
• It is seen 2-3 times more frequently in men.
• Patients may present with nen-specific symptoms, symptoms of cholestasis, recurrent episodes of cholangitis, or findings related to secondary biliary cirrhosis.
Diagnosis:
Cholestasis (ALP, GGT, bilirubin) markers are detected with MRCP/ERCP together with positivity.
• The appearance of rosary strings (stenosis and enlargement) in the biliary tract is typical on imaging.
• p-ANCA positivity can be seen.
Treatment:
• Supportive treatment is given for symptoms and signs related to cholestasis.
• Ursodeoxycholic acid can be used in medical treatment, but it does not slow the progression of the disease.
• The incidence of cholangiocancer and gallbladder cancer has increased in patients.
