Microscopic Polyangitis (MPA)


• In the past it was considered a variant of PAN. MPA is now recognized as a separate entity.

• Differences from PAN

It can cause glomerulonephritis.

o It can cause pulmonary capillaritis (alveolar hemorrhage).

it is ANCA ( + ).

Clinical Findings

• MPA is similar to granulomatous polyangiitis (GPA) in terms of lung involvement (pulmonary capillaritis) and kidney involvement (crescentic glomerulonephritis). However, unlike GPA;

o It is not expected to involve the upper respiratory tract.

o Nodular - cavitary lesions are not seen in the lung.

o There is no granuloma in the biopsy taken from the lesion.

o The majority of patients are pANCA positive.

Treatment

• It is basically like the treatment of granulomatous polyangiitis.
