Otitis Media With Effusion (OME)


It is the accumulation of fluid in the middle ear without signs of infection such as fever and pain.
The middle ear mucosa undergoes metaplasia and there is an increase in the number and secretion of goblet cells.
• It is most common at the age of 3-8 years.
• Allergies, viral and bacterial infections and eustachian tube dysfunction are to blame.
• The most common symptom is conductive hearing loss.
• There is no inflammation in the neck.

Membrane appearance in EOM
Increased blood vessels -The film is matte - Air bubbles behind the membrane - The disappearance of the triangle of light
treatment
Antibiotic - phlegm - decongestant - Adenoidectomy - antihistamines - Ventilation tube (lower anterior quadrant of the membrane) - Anti-inflammatory- tympanoplasty is not performed - New asymptomatic patients can be followed up without treatment
Indications for OME ventilation tube:
Retraction in the membrane 
Accompanied with sensorineural loss
speech delay
Hearing loss greater than 35 decibels
Frequent ear infection attacks
The presence of prolonged effusion
Entering the fall and winter season
Otitis media with effusion in adults
It most commonly occurs after an upper respiratory infection
All nasal diseases such as nasal polyposis, deviated septum, and sinusitis are causes of EOM in adults.

Otitis with unilateral effusion should be considered pharyngeal carcinoma until proven otherwise.
