Volkmann Ischemic Contracture

0 It is a deformity that develops as a result of compartment syndrome of the volar aspect of the forearm.

0 It usually occurs in children under the age of 10.

0 It can be caused by tight casts and dressings that prevent blood flow, immobilization at the elbow at an angle of more than 90 degrees, crushing of the forearm, arterial embolism and drug extravasation.

0 It is especially seen in supracondylar humeral fractures.

0 The most frequently injured vessel is A. brachialis.


Clinic

• It starts with a severe pain in the forearm a few hours after the trauma. The pain increases with the extension of the fingers.

• There is an increase in temperature, swelling and redness on the volar face.

• In a few days, the flexor and intrinsic muscles of the hand become paralyzed, and the sensation in the radial and ulnar nerve dermatome decreases.

• The volar aspect of the forearm stiffens and the sign of 5P appears:

 Pain (pain)

 Pulselesness

 Pallor (pale)

 Paresthesia

 Paralysis

• Contracture deformity remains in the form of metacarpophalangeal (MCP) joint extension, proximal interphalangeal (PIP) and distal interphalangeal (DIP) joint flexion, and atrophy occurs in the hand.

• Emergency surgical decompression is performed in the acute phase of treatment. In the late period, the contracture is tried to be corrected.
